Apply for Service

Please complete this form to begin the application process. A customer service representative will contact you during regular business hours by the next business day. You can also apply for service at any of our 6 offices during normal business hours: Monday – Friday, 8 a.m. to 5 p.m.

A $5 membership fee and a $20 non-refundable connect fee are required. A security deposit of $0, $200, or $400 will be determined by the credit rating of the applicant.

The following documents will need to be uploaded with your application:

  • Current photo ID
  • The purchase agreement, lease, or rental company form
  • A credit reference letter, if available (must be most recent 12 months; show no delinquents, disconnects, or returned payments; and name on the account must be the same as on this application)
  • For commercial purposes, we will need either EIN document from IRS; LLC documentation; sole proprietorship; etc. to verify the name of the business.
